Home > Programs > Continuum of ServicesThe mission for SAM’s Kids is to develop a continuum of services for children and families in need. The continuum is a complete range of services. By offering a complete continuum, clients can seamlessly move up and down the continuum as their needs either increase or decrease. The continuum will ensure that the client receives the appropriate level of service with sufficient support, coordination, consistency, follow up, and transitional care to provide the optimum chance for success. Case ManagementEvery child accepted into SAM’s Kids would be assigned a case manager. This case manager will remain with the family as they move up and down the continuum. The case manager will help ensure that the appropriate level of service is provided. The case manager will also help ensure the communication and consistency between all levels of service. Structured / High Intensity DaycareThis service would provide daycare services to elementary aged youth who were in need of more structured programming then currently available. The goal of the daycare would be behavior modification that would allow for children to transition back into mainstream daycare settings. We would be able to work with local childcare providers to accept their more challenging youth. The focus of the programming would be behavior modification and we would work on emotional intelligence and self-discipline. After School CBT ProgrammingThe CBT programming would be similar to structured daycare but set up to serve Middle and High School age youth. Cognitive/Behavioral Training classes would be the foundation of the programming. Recreational activities would be utilized to help the youth develop and internalize their newly learned skills in a social setting. Parenting ClassesParenting classes for SAM’s Kids would incorporate the same Cognitive/Behavioral curriculum utilized with youth in all levels of programming. It will help parents understand the programming and how it works. It will also help them develop interventions that will provide consistency for their child by teaching them to utilize the same techniques and terminology that is used in their SAM’s programs. Mental Health ClinicThe Mental Health Clinic will provide the Therapeutic, Psychological, Psychiatric, and Addictions services needed by our clients. High Intensity Wrap Around ServiceThis program will include the Structured/High Intensity Daycare or CBT programming. However, it will be a much more intensive program that will provide for one-to-one work with the youth at home, in school, and/or around the community. This will allow for more intensive practice of skills learned in programming. There will also be more intensive work with parents on interventions for their child’s unique needs. Included with this service would be a 24-hour crisis support line. Alternative SchoolFor children who have continued to be unsuccessful in the public school system we will provide alternative school programming. This service would be available to those students who need a consistency in service delivery to ensure internalization of treatment. Respite ServicesRespite is a short-term residential service that provides both the child and parents a break from their daily struggle. It will allow the child time to relax, reduce stress, and prepare for re-entry into their normal environment. It will also allow the parents a chance to regroup and reenergize so that they can more effectively implement effective interventions. This service can be for the day or overnight. Structured RespiteWhile the standard respite is designed simply as a break for parents and child, this service will provide more structure programming. Structured respite may be for longer time periods then regular respite but will still be considered a short term intervention. There will be less social and recreational activity within the structured respite program then with the standard respite services. Youth may be transitioned into standard respite prior to reentry into home environment. Foster CareFoster care services will be available for those youth who do not have a home available for them but are able to be maintained within the community. Foster care can be either short or long term depending upon the child’s needs. Group HomeGroup home services will be available to those youth who need long-term residential services but require more structure and supervision then foster care. Structured ResidentialHighly structured and secure residential programming for those youth who have been unsuccessful at every other level of service. Similar to the structured respite but designed as a longer-term intervention. For more information on any of the programming please contact jamie@samskids.org. |
